Foundational

Assessment

Do you wish that your annual exam was more than just a Pap smear and breast exam? What if your annual exam was actually a comprehensive, holistic picture of your health? What if you actually had time to discuss all of your concerns?

The Foundational Assessment visit is designed to do all of these things - it's what an annual exam SHOULD be.

The Foundational Assessment is the initial visit at MeadowLark and serves as a comprehensive onboarding experience. During this visit, we take a deep dive into your medical, family, and social history to understand how these factors have shaped your current health. We also explore your symptoms, lifestyle, biomarkers (blood work) and health goals to begin identifying the underlying contributors to how you’re feeling. From this, you’ll receive personalized lifestyle recommendations and a clear roadmap for next steps.

This appointment can function as a stand-alone consultation for those who only want an annual health assessment

Membership

Plans 

Our Membership Program is designed for patients who want a more supported, relationship-based approach to their care. This option is ideal for those with multiple health concerns, those navigating more complex symptoms, or anyone who values consistent guidance and a deeper level of partnership in optimizing their health.

Membership includes regularly scheduled visits throughout the year, allowing us to move beyond reactive, fragmented care and instead build a thoughtful, proactive plan tailored to you. With more frequent touch-points, we’re able to monitor progress closely, adjust your plan in real time, and provide ongoing support as your body and goals evolve.

This program is also required for patients participating in medically supported weight management. The use of weight loss medications requires careful monitoring, dose adjustments, and regular follow-up to ensure safety, effectiveness, and preservation of lean muscle mass. Membership ensures you receive the level of oversight and support needed for the best possible outcomes.

Fee-for Service

Visits

For patients who prefer flexibility, we offer traditional fee-for-service visits scheduled in time-based increments. This option works well for those who are generally stable, have specific concerns to address, or do not require ongoing, frequent follow-up.

30-minute visits are ideal for brief check-ins, review of stable conditions, or straightforward follow-up.

45-minute visits are appropriate for patients with new follow-up concerns, medication adjustments, or moderate complexity.

60-minute visits are best for new concerns, more in-depth discussions, or when a more comprehensive evaluation is needed.

To ensure each visit remains focused and high quality, concerns addressed will align with the time booked. If additional time is needed beyond your scheduled visit, a follow-up appointment will be required. In some cases, additional visits may also be recommended at the physician’s discretion to fully and thoughtfully address your concerns.

If you anticipate needing more frequent visits or ongoing support, our Membership Program may be a better fit, offering more continuity and a higher level of access to care.

Additional

Services 

In addition to the care provided through MeadowLark membership and fee-for service office visits, we also offer a range of additional services for patients who may need more specialized evaluation or procedures.

As a physician trained in both functional medicine and OB/GYN, Dr. Lewkowski also provides select in-office gynecologic procedures when clinically appropriate. These services allow patients to receive comprehensive, convenient care in a comfortable and personalized setting without needing referral to another practice.

Transparent pricing will be provided for all procedures and additional visits so patients can clearly understand costs before any service is performed.

Some examples of procedures that may be performed include:

  • IUD placement and management

  • Endometrial biopsy for evaluation of abnormal uterine bleeding

  • Vulvar biopsy

  • Other - discuss with Dr. Lewkowski as needed.
