Our office does not bill insurance. You may pay by check, credit card, or HSA debit card. Upon request, we can provide you with a copy of the superbill which you can submit to your insurance company for possible reimbursement for out-of-network care. We cannot guarantee what reimbursement, if any, you may receive. We do not submit claims or call insurance companies directly. This is the responsibility of the patient.
We are unable to see Medicare or Medicaid patients as federal law prohibits Medicare or Medicaid beneficiaries from paying out of pocket for services that these programs typically cover.
Unfortunately, when doctors participate with insurance carriers, they enter into a contract that allows the insurance companies to dictate what services will be supplied, limiting the quality of care that we believe you deserve. Additionally, insurance carriers dictate the payment that your physician receives for their services. As reimbursement rates have fallen, most doctors are forced to see more and more patients a day, keeping office visits very brief. We believe this approach of seeing patients for shortened visit times does not allow us to create the personalized plan of care that is necessary in a functional or integrative approach.
Some laboratory services provided by LabCorp or Quest can be billed directly to your insurance plans unless you don’t have out- of-network coverage. However, the out of pocket (cash pay) fee for our Foundational Assessment blood work typically runs around $250-300. This may be less expensive than if you use insurance to pay for your labs. Most have found that this cost is less than their copay for the same tests. It also gives us the freedom to order a more comprehensive workup without concern for a denial of coverage, as the labs are paid for upfront.
Some specialty labs may not be covered by your insurance plans. Please check with your insurance company regarding deductibles and reimbursement for out-of-network labs. It is not the responsibility of MeadowLark Functional Medicine to understand your individual insurance coverage.
A typical MeadowLark patient is someone who wants to better understand their body, feel their best, and take a more proactive role in their health. Many of our patients are women in midlife navigating concerns such as perimenopause, weight changes, low energy, poor sleep, or hormone-related symptoms, often after being told their labs are “normal” but they still don’t feel well.

Some MeadowLark patients come with existing medical diagnoses such as diabetes, hypertension, hypothyroidism, or metabolic concerns and are looking for a more comprehensive approach to their care. While medications may still play an important role, we work together to identify sustainable nutrition, lifestyle, and metabolic strategies that can improve overall health, enhance quality of life, and in some cases reduce long-term medication dependence when appropriate.
Our patients are often busy professionals, caregivers, or active individuals who value a personalized, comprehensive approach and want more time, access, and partnership with their physician than traditional healthcare settings allow. MeadowLark patients tend to be motivated, curious, and ready to invest in improving their long-term health. They are looking for root-cause answers, prevention-focused care, and a trusted medical partner to help guide them toward feeling stronger, clearer, and more resilient both now and in the future.
Our treatment focuses on lifestyle intervention, including nutrition, exercise, sleep, stress reduction and optimization of relationships. Herbals and nutraceuticals/supplements may be prescribed to help support your body, based on personalized needs. Prescription medication may be prescribed as well, depending on disease and symptom severity. This truly holistic approach incorporates both alternative and conventional options, and is overall directed by your goals and preferences; within context of safety and efficacy for the health concerns being addressed. A trusting relationship between the patient and her physician is paramount and an extremely valuable tool available to promote health and healing. Time is spent cultivating this relationship to build mutual trust and respect.
Although Dr. Lewkowski may provide some services that are traditionally considered primary care or gynecology (routine gynecologic exams, cervical cancer screening, IUD placement, etc.), you are required to also maintain a relationship with a primary care physician for after hours or urgent care as well as for conditions that may be outside Dr. Lewkowski's scope of practice. Although every effort will be made to respond to portal messages in a timely manner (within 24-48 hours), we may not be able to respond to urgent requests, including messages or phone calls. It is important that you have a primary care doctor for this purpose. Also, Dr. Lewkowski does not provide surgical or hospital care. Dr. Lewkowski does not provide obstetric care during any trimester.

Membership also provides the structure and support needed for programs like medically supported weight management, which require more frequent follow-up.
Fee-for-service visits are a good option if you are generally stable and prefer to schedule appointments as needed for specific concerns or periodic check-ins. This model offers flexibility and works well for patients who do not anticipate needing frequent visits or ongoing support. In general, Dr. Lewkowski recommends that patients who are receiving prescription medications be seen every 6 months.

Our Medical Weight Loss Program is designed for patients who would benefit from additional metabolic support alongside nutrition and movement recommendations. This program combines evidence-based medications with personalized lifestyle guidance to help improve body composition, support metabolic health, and promote sustainable weight loss.
Patients who are utilizing weight loss medications in this program receive ongoing body composition assessments to help ensure that weight loss comes primarily from fat mass while preserving lean muscle. Regularly scheduled visits (every 4-6 weeks) allow for close monitoring of medication response, management of potential side effects, dose adjustments when needed, and continued coaching to support long-term success.
This structured, physician-guided approach helps ensure weight loss is safe, effective, and aligned with your overall health and longevity goals — not just the number on the scale.
Please note that as regular follow up visits are necessary, enrollment in membership is required for patients utilizing weight loss medications.
